Chemistry Field of Concentration Forms
Note: Chemistry-specific forms are also available from the Department of Chemistry office, room CB 213.
Field of Concentration Forms are checklists for completing chemistry major requirements. They are to be reviewed with your Chemistry Department faculty advisor and filed with the College of Science dean's office at the beginning of your junior year.
Select the form for your degree option and General Catalog year. You may choose the catalog year of your (i) initial enrollment or last change of major (whichever is later) or (ii) year of graduation. See the General Catalog for details on your applicable catalog year. Forms are in pdf format:
- Bachelor of Science in Chemistry with Professional Chemistry Option
ACS certified program (Program code BS-CHP) - Bachelor of Science in Chemistry with Environmental Chemistry Option

- Change of Major/Additional Major/Degree Declaration
- If the program code on your Degree Audit (DARS) Report does not match the one above for your degree and option, you should also file the Change of Major/Additional Major/Degree Declaration form.
- Request for Major/College Substitution/DARS exception
- Course substitutions for the major program must be approved by the Chemistry Department Chair, and filed using the Request for Major/College Substitution/DARS exception form.
- Request for Core Substitution/DARS exception
- Course substitutions for the University of Nevada, Reno core curriculum requirements are filed using the Request for Core Substitution/DARS exception form.
